Due to a variety of factors, preventable transfers from skilled nursing facilities (SNF’s) to the hospital emergency departments (ED) are recognized as creating unnecessary expenses and stress for residents and staff. This session will review a pilot program involving six SNFs in the Southern Tier of New York, where telehealth was used to reduce preventable ED transfers by nearly 30%.
